I am translating a text regarding a person who gets in with the wrong crowd, the outcasts of the area, plays cards and gambles etc. This person, highly educated, attaches nicknames to many people he meets, and I am having trouble translating these. The problem is the difference between Italian and English in this respect - Italian manages to be so eloquent and almost cultured while being vulgar (!), while a literal translation would not work in English, we are so much more direct :-) Would you leave the Italian and give an approximation in English, or give an expression which an English speaker would use in such a context with reference to this person? Any help would be much appreciated. Thanks!

I'd definitely give an English equivalent instead of leaving the Italian word. This is my humble attempt - I think the person described in the text sounds like what we used to call a "perv" at school :)

The expression actually does not exist in Italian, it's made up here, so I would avoid common/current English expressions.
It does sound quite elaborate and "learned" since *scroto* is almost a medical term.
I would go for a literary translation.

at least here in the U.S., that would seem to be the most natural epithet for a "letch", which would be another possibility

: I would definately translated, if you are looking for a word to use for all audiences "pig" would work well. If you are looking for a more adult term you might use "asshole", "scum bag" or something of this nature. Audience is the key...

I just want to draw your attention that the name is in quotes so it has to sound like something that would roll off the tongue, a nickname and not an epiteth.
I am not a specialist in slang, so this is the best I can come up with.

or something like that. Actually as it has been said, "scroto" is almost medical, and you told us this is an educated person. In fact, "scrotone" is made up, nad make me laugh. I don't know why "crudele" but maybe you know it from the text. We all know this voyeur can be a "pig", but the original expression conveys much more, well actually it doesn't imply voyeurism at all. So its gets lost in translation and it's too bad. Or "big bad balls".. for those who love alliteraions and acronyms;) BBB Mean bollocks?;)

It's a just-for-fun suggestion, but I do think the Italian pun is from "scroto" and "scrutare, scrutinare", since this jolly fellow spends his time "scanning, scrutinizing" those very young chicks.
